To become a participant in the Queensland Off-The-Track (QOTT) Subsidised Lessons Program, the person who will be participating/riding in the lessons needs to complete the form below. NB: The applicant's name and details entered on this form must be the person who will be participating in the lessons.
Please ensure you read the Eligibility Criteria; Terms and Conditions; and other relevant information about the Subsidised Lessons Program prior to completing the application form at: https://www.racingqueensland.com.au/off-the-track/subsidised-lessons

The Queensland Off-The-Track Subsidised Lessons Program Terms and Conditions govern the participation of you as a Participant in the Program, or involvement in the Program as a Guardian of a Participant under eighteen (18) years of age. If approved to participate in the Queensland Off-The-Track Subsidised Lessons Program, QOTT will issue me with a copy of the Terms and Conditions via email. I acknowledge that I must sign and return a copy of the Terms and Conditions to QOTT within (1) month of approval by QOTT to participate in the Program or my participation in the Program will be cancelled. For Thoroughbreds, two photos are required (i.e. one photo of your horse's offside shoulder brand and one photo of your horse's nearside shoulder brand).
For Standardbreds, one photo is required (i.e. one photo of your horse's offside neck brand).
NB: Photos of your horse's ID card will not be accepted.

Applicants must contact their preferred Approved Coach prior to nominating the Approved Coach on the application form to confirm the Approved Coach’s availability, suitability and location for where the lessons will be held. Participants are not permitted to change their Approved Coach once the application form has been submitted. The list of Approved Coaches and their contact details are listed on the QOTT website here https://www.racingqueensland.com.au/off-the-track/subsidised-lessons.
PLEASE NOTE: Approved Participants are issued with horse coaching lesson vouchers for use with the Approved Coach nominated on the application form. Participants are not permitted to change their Approved Coach once the application has been approved and all horse coaching lesson vouchers must be used with the nominated Approved Coach.
